Haiti Earthquake Declaration Form Now Available

In our earlier news release about the Haiti Earthquake Matching Contributions, CCCC advised that charities will be required to provide a declaration form in order for donations to be taken into account in determining the size of the Haiti Earthquake Relief Fund to be administered by CIDA.

CIDA Declaration Form Now Available

This 2-page form is now available and is fairly straightforward to complete.  This form is not an application to receive grants from CIDA, but a declaration of donations received.

Charities will need to ensure:

  • that the form is received by CIDA on or before February 26, 2010
  • that the form is sent by fax, registered mail or courier to the address on the form
  • that an authorized representative signs the form
  • that the amount declared includes eligible donations from individuals between the period January 12, 2010 and February 12, 2010.  Eligible donations are defined on the form.
  • that the amount of eligible donations declared excludes donations already declared by other organizations.  (For example, if a church transferred funds to another Canadian registered charity to do the work, the church and the agency need to coordinate and ensure the eligible donations are only declared once.)

Allocation of Matched Funds

CIDA will allocate the matched funds on behalf of the Government of Canada.  Decisions for funding will be made based on the merits of the proposal and the ability to deliver aid quickly and effectively.  The organization will need to have significant in-country capacity to deliver humanitarian and early recovery assistance.

Charities receiving the initial donations will not necessarily receive the matched funds.
